The envelope in which this letter is contained bears the St Mungo's seal stamped onto it. Inside is a piece of  parchment with the St Mungo's logo as the letterhead. The message is written in dark green ink, with a neat, rounded hand.

Dear Mr Spectre,
Please be informed that this letter contains health information strictly confidential to you and kin related to Mr Johann Storm, admitted to Potions and Plant Poisoning on 4th December 2010 at approximately 11.40pm under the diagnosis of "contaminated sleeping potion poisoning". Listed below are what his healer requires you to do post-discharge in order to improve his health for the better outside of the hospital.

The patient cannot be allowed any amount of caffeine or alcohol immediately after being discharged for at least one week. After that, alcohol may be reintroduced, but only at one unit/one small glass of red wine per week. That should be the maximum permitted. Please keep tight control over this.

Mr Storm will need to eat the usual breakfast, lunch and dinner at specified times of the day. Please ensure his diet is balanced and has plenty of water, green vegetables and fruits present. To alleviate stress on his kidneys, red meat can only be taken once a week, and meat in general, including fish, up to three times a week. He also cannot consume spicy or rich food for a week. Please ensure that he is properly hydrated.

He must keep taking his sleeping potion as per his regular schedule, but in gradually diminishing amounts every four to five days. Getting rid of his reliance of Somniculose should be the first goal to cover. He can continue consuming it for this week, but then must gradually shift back to hospital-regulated sleeping potion. Please inform him that Healer Hollingbury will personally throw a book at his head should he find traces of Somniculose in his blood past that one week of allowance.

Johann Storm is strictly required to be resting between the hours of 10pm to 7.30am daily, with no work or distractions to keep him from sleeping. He should not be permitted to take naps throughout the day either.

He is permitted to have sick leave from the Ministry for at least one week. A letter has already been sent to his department head informing her of his time off work. He is also free to take sick leave for an additional two weeks if necessary. If he chooses that option, please inform the hospital so that Healer Hollingbury can issue another healer's note to the Ministry.

Should any issues and/or questions arise, please contact the hospital immediately to consult with any available healer if Healer Hollingbury is not available. Please inform Johann that he has been booked for a 2pm appointment with his assigned healer on the 14th of December 2010 for further check-ups as necessary.
